When I click on the Download Manager, the authorization dialog appears ("Do you want this application to make changes", etc). I click "Yes" on the dialog, then nothing else happens.

You should run it from the Band-in-a-Box Help menu | Utilities ... is that what you're doing?

Oops. I ran it as a stand-alone.

Running it from BiaB->Help->Utilities is what works.
