I don't recall this being a problem before BIAB 2019 -- it didn't affect my install anyway. Version 604 installed.
Now -- if you go to "MIDI Driver Wizard" tab in the audio control setup dialog box: There are 2 choices with radio selection buttons. "internal Speakers" "External Speakers (via indicated MIDI connection, another button)"
If you select the SECOND ("External") and then the MIDI device this holds at times, but upon restart of the program it reverts as default to the FIRST ("internal") which typically uses MS Wavetable. Also seems to go back and forth under some MIDI driver changes.
Prior to 2019 I had no such difficulty. I use ASIO drivers.
This appears the case in either the 32 or 64 bit versions.
The above wizard is about setting a non-VST/DXi synth as a playback driver for MIDI. The image below is the information I obtained when I pressed the "Help" button in the "MIDI Output Driver Wizard".
ASIO is an audio driver (not MIDI). Also, since I suspect that you run your MIDI sounds through a VST Synth such as Coyote WT or VSTSynthFon64 (in BIAB 2019). There is little to be gained by using the wizard.
For what it's worth, and I'm not a 100% sure that this is still the case, the VST Synths work best when "MS GS WaveTable Synth" is selected as the output MIDI driver (even though the VST synth is the one doing the driving).
If you have a look at the below link, you'll see the basic setup that I have on my computer for these settings. (Only the numbered settings are relevant.)
I thank you, friend, for the valuable insights. It turns out that my particular setup called for some slight changes here and there. I did test using the VSTdi setting (64 Soundfont) and that with MME did work well.
In the process I discovered that my Forte Ensemble installation was done with a setting of 48Khz and not the 44Khz that BIAB requires to sync, I believe. Changing this to relate to BIAB removed all the squeaks, stops, and general weirdness in my 2019 BIAB install.

Handy flash drive tip: Always try plugging in a USB device the wrong way first? If your flash drive (or other USB plug) doesn't have a symbol to indicate which way is up, look for the side with a seam on the metal connector (it only has a line across one side) - that's the side that either faces down or to the left, depending on your port placement.
