I've made a backing track, with an audio file of playing the accordion on top.
There are some tempo changes towards the end, where the piece slows down.
The piece plays at the tempo I want in BIAB - the problem occurs when I save it to mp3 or wav - then I hear the backing track playing at the original tempo, without slowing down, and me on the accordion playing at the tempo I want.
"Save songs with patches and harmony" is checked.
What else can I do to make the mp3 play at the tempo specified in BIAB?

What version of Band-in-a-Box (BiaB) are you using? Go to Help > About Band-in-a-Box and the version and build numbers are displayed. The latest updates are available +++ HERE +++.
What steps are you taking to save the song file to MP3 or WAV audio?
I have laid out my chords, and using style ZZWaltz.Sty, for a simple backing track. The tempo starts at 130 bpm, and slows down by 10 bpm in three bars towards the end, so finishes at 100 bpm.
I have then recorded myself playing accordion onto another track.
The complete tune plays back perfectly, within BIAB, complete with slowdown at the end.
I've then tried Save As Special... to mp3 and wav.
Each has the same result - the accordion plays at the speed I played at. The backing track does not reduce tempo where specified.
This is a different approach, but I think will work for you.
Click-Hold and Drag the Master track and drop it onto the MP3 target (your version might have 4 targets, not 6)
After the file renders, the target will change to Green color. Then check in your \bb\DragDrop\ folder for the .mp3 file (I'm thinking that was where it was saved to in 2016, but you may have to search for the file).
I just tried this method and the tempo change definitely was produced.
