Do you have the GS Wavetable or MIDI Mapper selected as your MIDI Output Driver? Sometimes ASIO4ALL and the Wavetable conflict, which stops ASIO4ALL from working properly.

This is usually fixed by selecting a different MIDI Output Driver. If you don't have any other options, the (free) Loopbe1 driver usually does the trick. You can get it from http://nerds.de/en/download.html

Just make sure you're using a DXi synth, or MIDI output will be silent.
