As I mentioned when I started today, I've been going down the UT list from newest to oldest, so maybe you know this already:

You need to be _very_ careful about what is happening at the first beat. If your note(s) from the bar before that is still being heard (overlaps), then it 'bleeds' into the chord change if there is one at the first beat. And this becomes most apparent if the chord also needed a pitch change by the BBox, because then the overlapping sound from the chord before gets pitch changed too. This sounds particularly bad on instruments like acoustic piano or others where notes do not bend like they can on guitars or such.
Another (and perhaps more plausible) reason for those pitch issues could be a slight latency in the VST interface, so your resulting audio track is a tiny fraction behind the beat. This can be fixed by shifting the track back a few ticks _before_ you then save it as a Usertrack.
