Mono is NOT checked! I'll bet that is your problem.

Mono, in this context, means one, as in one note. So if you want more than one note (a chord), you need to uncheck the box.
Clean, in this context, means that the display should show a clean look, even if (as often happens with live playing) the notes are a 16th or 32nd note off the beat). That way, the note displays cleanly on the beat, even though it was played just a little off the beat. Of course, in this case, you can use the quantize feature to permanently push things back to the beat.
