Proper results? Not often. Some results, possibly usable? Sometimes.
I don't know what you want to do with the MIDI. The following comments have to do with scanning PDFs to create music notation. I use Music XML format for that. It can then theoretically be imported into BIAB (which I haven't tried), or into music notation programs.
About importing into music notation programs, success is entirely dependent on the quality of the original. Even with perfect originals, I often have to do tweaking.

Was experimenting with a downloaded PDF of a fiddle tune simple lead sheet, i posted it here in case you want to see it: Bridge over the river Ash. Once recorded by Fairport Convention .... a fiddle pupil wanted to learn the thing, and was trying to get BIAB to create a backing-practice track as Mp3. Importing audio in BIAB of a whole band's recording doesn't work ... F
Again, not cheap, but there is a free download to try out and see if it does what you want. As others have said, it very much depends on the quality of the original.

The chords are not there (only two tones) but are easily inferred if you want to have other instruments play it in BIAB. Do you have the skills to do that?
And one drum hit at the end ? Funny.

Well, i was doing it as an experiment, sort off, but it has been recorded by Fairport Convention, with a quite innovative bass part by Dave Pegg, their bassist back then. Met Peggy once later in life, through a mutual friend. F
