After the update from build 1004 to 1006 my Utility RealTracks have a MIDI piano on the right channel when I generate the complete song. When I re-generate the individual Utility-tracks via 'Track Actions -> (re)Generate this RealTrack' it's okay again ... until I generate the complete song again.
Edit: when I render the song to disk there's NO piano, it's only there while listening in BIAB.
Edit 2: how to reproduce (and hear!) the problem
start BIAB
select style _FLIPSID.STY
generate song (there's no need to enter any notes)
add RealTrack 3931 as first Utility track and pan it to -63
regenerate the complete song
listen to the Utility track in solo
BTW, I did reset all preferences to the factory setting before posting here.
You say there is a midi channel being sent to 6 utility tracks?? That is odd indeed. Can you upload the .sgu/mgu file?
Regarding Utility#3. There is a new 2023 feature found in options-preferences-Output Chords. This feature will automatically write the chord chart as a midi track to Utility#3. It should be off by default, but if it is on you may hear the chart chords playing via you default synth through your song on this utility track.

Thank you for the information, I appreciate this a lot. I did reset all BIAB settings before posting this bug report, however I double-checked Output Chords and it's definitely OFF.
BTW, I've added a How-To to my original posting, so everyone should be able to reproduce the problem.
I can't reproduce it following your steps but I have had this very (or similar) problem with ghost piano parts playing, not just this version but also 2022 and possibly 2021 (can't remember when the first time I had it happen but definitely pre-2023).
Unfortunately I can't remember exactly what the problem was but it took me a lot of messing about.
It was somewhere buried in midi channels or something, just another one of those oddities in BIAB where nothing makes sense or stays consistent. The track settings window, the various right-click menus all around midi channels or routing or something like that, which are supposed to affect the same parameters, but didn't correlate and wouldn't stick to what they were set at. Nothing to do with PGM or anything else you'd find in the list editor.
Creating a new song didn't get rid of it, closing and reopening wouldn't reset it but loading random styles sometimes did. I wish I could remember but sorry. Just endless trial and error solved it in the end.
Is this perhaps related to some Utility (especially guitars) tracks using MIDI Channel 1 for Notation?
In my case, my external synth is receiving (utility guitar) MIDI on Channel 1, so I will hear both RT Guitar and synth MIDI audio when BIAB is playing.

Is there any setting to disable this?
Check your preferences. There have been reports that these have changed from the defaults.
Under 'Preferences -> MIDI Driver' I had always my master-keyboard as 'Input Driver' and Microsoft GS Wavetable Synth as 'Output Driver'. When I unselect the latter and chose '<No MIDI/sound output>' the problem is gone.
It's still a bug, but at least I have a workaround now...it was a bit tiring to regenerate each track by hand each time...

Hey, I just found that you can set a MIDI channel on a Utility track!
Right-click on the realtrack, select Track Settings and MIDI Channel. Select a MIDI channel and the utility track will play both realtrack and MIDI on your selected channel. (Which makes for some nice layering...).
If you don't want this, try setting "Do not re-route (multichannel).
MIDI is still wonky, but now I can use my piano on channel 1.
