I tried a return to factory settings as Barry suggested but still the chord would not play. I played around with the MIDI driver settings and found that the chord would only play when I set the MIDI output to Microsoft GS Wavetable Synth so problem solved. The song still plays through Forte if "Use VSTi/DXI Synth" is checked or plays Realtracks if those are generated. Judging from the low quality piano sound I hear when playing the chord it seems to be coming from the Microsoft GS Wavetable Synth.
