An old Scottish ballad goes

Did kill the Earl of Warren
And laid him on the green.

The woman who later coined the expression had in her girlhood misheard that as

Did kill the Earl of Warren
And Lady Mondegreen.

In adulthood, she finally saw it written out, and her term for it has stuck.
