Since it's MIDI, it runs with a clock.

In BiaB, I do believe you can edit the tempo on a measure by measure level. Of course, you set the project tempo and if it's not changed the song will stay at that tempo throughout.

I recall that you can click on the measure and set the tempo to something other than the project tempo for that one measure. You can edit the other measures around it the same way to have a longer period of different tempo.

For a synth to stay synced to a BB project track it would have to be watching the tempo clock..... so if you change that in a measure the synth should follow.

I don't ever change tempo in songs so I'm going from what I recall from way back when I first read through the manual.

I don't believe you can edit the tempo in Real Band. I might be wrong though.
