The first one is the default choice, the second submenu is a menu of choices.

For example, if you use the submenu to save as a .m4a, then the next time you use the menu, the m4a option gets promoted to the main menu, and you don't need to navigate through a sub menu to find it.

For saving as mp3, it uses built-in codecs that come with Windows. Those have changed with different versions of Windows, and you can obtain others on the Internet.

m4a is a good option to use, it is the successor to mp3 (and was called mp4 for awhile).
