I have tried output through both my MAudio Audiophile 2496 PCI sound card and my Mackie Onyx blackjack USB recording interface. Both have the same result. Noise and distortion from RealBand playback, but not in any other audio related application, including Band In A Box. So adusting buffers is not the answer since everything else is working.

Also, I tried rendering a WAV out of RealBand. The WAV file does not have any of the noise or distortion. This seems to be a playback only problem affecting RealBand only.

Remove ASIO4ALL from your computer and load, update and use the native ASIO driver that's supposed to be used with your soundcard/interface.

Disregard what other programs do. Use the proper drivers for them all. ASIO is what you need.

Understand that A4A is a wrapper and not a true driver. It fools MME into thinking it's ASIO and ditto on the hardware. But in many cases, some hardware isn't fooled and A4A doesn't work. I've never been able to get A4A to run on my computers at all.

It's also possible that the other programs are not using A4A but are defaulting to MME and will run fairly well on them in playback mode.

Unfortunately, ASIO is a single program at a time driver. So whatever grabs it first gets it. If you then open a second or third music program, they will not be able to use ASIO and will default to MME.
