Hmmm, I thought that might be it Matt, but by setting bar 1 to 1 beat it plays the first beat of the bar (which is a rest), when the pickup note is actually in the 4th beat of the bar.

I also tried inserting a bar before bar 1 to push the pickup note into bar 1 rather than bar 0, but that didn't work either (actually made the score read worse).

In this case, the pickup note is of course prior to bar 1.
