I generally use RealBand (although I have PTPA2017); however, I believe the RealBand updates are a little ahead of the PTPA updates, so maybe a forthcoming update will fix the problems.
I presume you've tried the obvious things, Paj, such as reinstalling, resetting your audio settings, running as administrator, trying a different audio driver, ...

Band-in-a-Box 2026 Video: AI Stems & Notes - split polyphonic audio into instruments and transcribe
This video demonstrates how to use the new AI-Notes feature together with the AI-Stems splitter, allowing you to select an audio file and have it separated into individual stems while transcribing each one to its own MIDI track. AI-Notes converts polyphonic audio—either full mixes or individual instruments—into MIDI that you can view in notation or play back instantly.
