The first two guitar chords are probably called Dsus2 and C-add2, but I'm no expert on naming chords.
The first chord is basically a simple D chord with the first string open (ie. DADE).
The second chord is a simple C chord, but with D instead of C on the second string (ie. CEGDE).

When I play the song fingerstyle on solo acoustic guitar, I use a capo on the 5th fret and play A5 (AEGB£) followed by G6 (GBDGDE) chord shapes.

I played around with a few waltz styles before sticking with the PFPBTMW one. I love that double bass, and it really makes the song sound special, in my opinion.
