Proper results? Not often. Some results, possibly usable? Sometimes.

I don't know what you want to do with the MIDI. The following comments have to do with scanning PDFs to create music notation. I use Music XML format for that. It can then theoretically be imported into BIAB (which I haven't tried), or into music notation programs.

About importing into music notation programs, success is entirely dependent on the quality of the original. Even with perfect originals, I often have to do tweaking.

Two catches:

1) It's expensive.
2) It only seems to work on scores by established publishers or high-end music notation software.
