Sforzando is unable to be used in the default synth slot because it can only have a single session going (i.e. it can only be on one track at a time). All BIAB tracks access the default synth so it needs to contain a synth that supports this.
For the above reason, the default synth position needs to use a synth like TTS-1 or coyote WT that can have multiple instances.
With BIAB, sforzando is used for the MIDI Supertracks.
It's possible to have multiple instances of Sforzando running -- say a Piano MIDI Supertrack and a Bass MIDI Supertrack. This loads the VST on two individual tracks and so each track is only using the single channel that Sforzando has.
Where it falls down in the default position is that it cannot handle multiple tracks accessing a single instance of Sforzando. It's like 7 hammers (BIAB tracks) trying to hit one nail (Sforzando plugin) all at the same time.

Just load Sforzando on the track that has the guitar notes that you want to play. Once the Sforzando VSTi is loaded, then load the SFZ voice into Sforzando.
To load the VSTi, it's necessary that the BIAB track is a MIDI track (i.e. it is coloured yellow).
Now... go to the mixer and follow the 1, 2, 3... on the image below. I have loaded Sforzando on the Guitar track as an example.
Once that is done, open Sforzando and import the sound (or drop the sound in the drag 'n' drop the sound in Sforzando's main window).
