My health care plan is to eat, drink, smoke, carouse, and do whatever I like, then die.

Nice romantic, rugged individulistic type attitude there but what happens if you trip down the stairs in front of your house, break your neck, your neighbor calls 911 and you wind up in the hospital for 6 weeks and make a full recovery at a cost of oh, say 100 grand?
So, I guess that means everybody has to know you don't want anybody to call 911 on your behalf, you're willing to simply die there lying on your stairs in pain with a broken neck, right?

If you don't provide for your own insurance either through work or this new program, who pays for it?

C'mon man, think it through.
