I was plagued with this issue for years. Fortunately, with the new-ish Partial Track (Re)generation feature, I've found that if I highlight the section with the glitch (and, depending on the type of RT, instrument, or phrase, a measure or some beats before and after the transition that's glitching) I can finally get something smooth after a half-dozen or so regens.
