Not sure what went wrong, but am wondering if you can also HEAR those mishmashed notes, or is that just happening in the Notation while the music plays back just fine?
Also - What do you see in the main Notation Window view when this occurs? (In other words, looking to see if this is just a Lead Sheet bug or is it systemic throughout).
If you are using the new 2014BB and latest build (which is now at Build 378), you might want to attach the file in an email directly to PGMusic for evaluation faster and proofing to see if this is a real bug or what. Reference the URL to this thread.
